Description
Transgender women and men are at markedly elevated risk for HIV. This session reviews a successful four-pronged approach to building the capacity of a large multi-site RWHAP-funded community health center network, in collaboration with a community-based organization, to improve delivery of HIV primary care to transgender women and men.
Learning Objectives
- Discuss factors contributing to HIV risk in transgender populations which may also impact adherence to the HIV care continuum.
- Describe the successful collaboration between the community health centers and the community-based organization which led to a greater than 100% increase in engagement of transgender persons in HIV primary care.
- Review best practices for systemic changes to better engage transgender persons in HIV primary care.
